Student Health Services located in the Plageman Building at Oregon State is a wonderful resource for dealing with the aches, pains, and illnesses that often come with being a graduate student. Student Health Services also has insurance options for individuals and families. If you have a .20 FTE or higher Graduate Assistantship, you are required to be on the OSU insurance plan. For more information or for waiver information, go to the Student Health Insurance link. *Note: Every OSU student must complete and submit a “Health History and Immunization Form.” Failing to complete these forms could lead to holds on your student account. If you have not done so already please got to the Student Health Center Website, click on “Downloadable Forms”, print off the “Health History and Immunization Forms” complete them and mail or fax them back to the appropriate address.
